E-Scripts Prescription Service:

Prescriptions issued by E-Script are provided by GMC registered doctors. All prescriptions issued will have the name of the providing doctor along with his GMC registration number. The service is for licenced independent pharmacies only. You agree that all the information provided by you when opening an account with us is accurate and truthful. You agree that medicines dispensed against prescriptions that we issue will be dispensed in line with UK pharmacy guide lines and will only be licenced UK products. When you submit your patient’s personal information and medical history you agree that you will take all reasonable steps to ensure that the information given is accurate and truthful.

When requesting a prescription from us on behalf of your patient they must provide accurate personal and medical information. We do not accept responsibility for any prescription issued if your patient has provided inaccurate information with regard to their personal information or with regard to their responses to the medical questionnaire provided.

Your patient will be required at the end of their medical questionnaire to read and tick a box confirming he has read and agreed to our indemnity Statement which includes a patient disclaimer warranty and declaration in order to complete their prescription request. It is your responsibility to ensure your patient reads and understands this statement. See Indemnity Statement below.

In the case of Vaccinations our doctor will if appropriate issue a prescription for vaccinations. It is your patient’s decision as to where he has the vaccination. If it is to be administered in your pharmacy the responsibility is yours to ensure you are properly licenced to provide such vaccinations. You will take full responsibility for any adverse reaction that may occur following the vaccination and responsibility for the on-going care of the patient should they have suffered an adverse reaction.

Indemnity Statement (Patient Disclaimer, Warranty & Declaration)


I hereby release E-Scripts the dispensing pharmacy and all of their employees and contractors including physicians from any and all liability whatsoever associated or connected with my prescription medications request and/or use of those prescription medications. I hereby state that I am an adult and that I am aware of the potential side effects associated with prescription medications. I understand that no doctor, nurse, pharmacist or administrative personnel can guarantee that prescription medications, even if prescribed, will provide the results I seek. Further, I understand that even if prescribed, I may suffer adverse effects from prescription medications. I hereby release E-Scripts the dispensing pharmacy and all of their employees and contractors including physicians from any and all liability whatsoever associated with any adverse effects I may suffer from my use of the prescription medications provided. I am participating in this programme at my own choice, at my own expense and my own liability and assume all responsibility for my use of any prescription medications provided. I fully understand that it is my responsibility to have an annual physical examination, including any suggested laboratory tests, to ensure that I have no disease(s) which might make the use of prescription medications inappropriate for my condition. I further agree that I have consulted with my physician and/or the dispensing pharmacist and hereby warrant that I am not taking any medications or combination of medications that are on the published list of medications which would make any prescription medications provided contraindicated. I further agree to immediately notify any doctor whose present care I am under that I have chosen to take the prescription medications provided so that they may advise to continue or discontinue use. I wish to have prescribed and supplied to me branded product that is as inexpensive as possible, without prejudice to the other factors that are to be considered by those prescribing my pharmaceuticals and those fulfilling the prescriptions.


Certification and Warranty of Patient.

I hereby certify and warrant that I am an adult and have carefully read and truthfully answered all of the medical questions in my medical consultation. I further certify that I have completed this application with the purpose of employing the service of the E-Scripts doctor and that he will be relying on the truth and accuracy of my answers in determining whether I should have the requested prescription medication supplied to me. I understand if I have failed in any way to furnish the E-Scripts doctor with my complete and accurate medical history I have therefore not fulfilled my legal obligation to properly inform the doctor. I understand that if in the future my medical circumstances change in contradiction to the information I have provided that it is my legal responsibility to immediately notify the doctor or dispensing pharmacy and cease all use of the prescribed medication until further notification.


Patient declaration.

I am at least 18 years of age. I am permitted by law in my country of residence to receive the medication(s) I am requesting. I have had a recent physical examination by a physician who is available for any necessary follow-up care and intervention. I have been fully informed and understand the risks, benefits, and possible side effects of the prescription medications I may request. I am requesting the prescription medication(s) solely for my therapeutic and medical needs, and will not distribute any medication to others. I certify that I will use this prescription medication for, and only for, the prescribed use, and that I will not use it in conjunction with any illegal substance. I will promptly contact a local physician for any necessary medical intervention should a complication or concern arise as a result related to the use of a requested medication. I do not require a child safety cap on my medication(s) if prescribed. I have and will answer all questions truthfully, for my safety, just as I would with my own doctor. I wish to have prescribed and supplied to me branded products that are as inexpensive as possible, without prejudice to the other factors that are to be considered by those prescribing my pharmaceuticals and those fulfilling the prescription. I understand that if I am resident outside the European Union that I will be responsible for any customs, tariffs, and taxes, that may arise should I take this medication home. I certify that the foregoing statements made by me are true.


Privacy & Data Security:

We use state of the art encryption methods to ensure we keep your pharmacy details and the details of your patients secure. However, unless we are negligent we will not be liable for unauthorised access to information supplied by you.

This privacy policy sets out how we use and protects any information that you give us when you use this website. We are committed to ensuring that your privacy is protected. Should we ask you to provide certain information by which you or your patients can be identified when using this web site you can be assured that it will only be used in accordance with this privacy statement. We may change this policy from time to time by updating this page. You should check this page frequently to ensure that you are happy with any changes we may have implemented.

You may request details of personal information which we hold about you under the Data Protection Act 1998. A small fee will be payable. If you believe that any information we are holding on you is incorrect or incomplete, please write to or email us as soon as possible, at the above address. We will promptly correct any information found to be incorrect.

Payment & Pricing Policy:

The price of a prescription is £10.00 for each individual patient. An extra product within the prescription category may be added at no extra cost to the prescription. For example E.D prescription 1 box of Viagra and one box of Cialis would comprise of one prescription for one person at £10.00

In the event that a patient requires more than one prescription category on one prescription for example one ED product and one Anti-Malarial product this will be deemed as two prescriptions (even though it is issued as one) at a cost of £20.00.

In providing payment card details within your account, you confirm that you are authorised to use the card and authorise us to take payment in full for the applicable prescription charges and any other charges that become due to us under these terms. Refunds, if applicable, can only be made to the card originally used for payment.

You are required to open your account with a minimum payment of £200.00 there after your account will be deducted at a rate of £10.00 for every prescription issued. Once your account falls to £50.00 it will be automatically topped up by £150.00 to the £200.00 level. If you anticipate requiring a high level of prescriptions or you have a number of sub branches to your business you can set your minimum payment to a higher level of your choice.

 We want you to be delighted with our service. If you are unhappy with our service for any reason you wish to close your account any funds remaining in your account will be refunded in full.

IMPORTANT: E-Scripts will not accept liability for any loss or damage, in contract negligence or otherwise, where there is no breach of a legal duty of care owed to you by us, or the loss or damage is not a reasonably foreseeable result of any such breach or any loss or damage or increased risk of loss or damage resulting from a breach by you of these terms and conditions.

In particular we are not responsible for:

Loss or damage caused by you providing inaccurate or incomplete information;

Loss of your emotional well-being including, but not limited to, any embarrassment caused;

Loss of income or anticipated profits;

Loss of opportunity;

Loss of goodwill or injury to reputation;

Losses suffered by third parties; or any indirect, consequential, special or exemplary damages arising from the use of the service regardless of the form of action.

Nothing in these terms and conditions excludes or limits our liability for death or personal injury caused by our negligence or for fraud or fraudulent misrepresentation. Nothing in these terms and conditions affects your statutory rights
